Update the Add Team Member process to allow the user to add a note to the mentor they are adding on the mentor request

Mentors can add a team member from anywhere in the country to a client's request. The newly added mentor is then able to easily find, view, and add session to this request. With this change, the mentor adding the team member will also be able to add a note to the person they are adding to the request.
